You can "combine memberships" up to 3 memberships total on the original, primary offer. After that, any upsells or downsells are done independently.  Combined memberships do not apply to upsells/downsells. 

You can create a membership, and at the bottom of edit pricing step and delivery, check to add physical delivery and add your physical book right there. If the book is digital just deliver it in a membership as a download.
